Dental Insurance
Our office is experienced in managing dental claims for all dental insurance plans. We will help you to understand how to maximize your benefits and minimize your out-of-pocket expenses whenever we can. Dental claims are submitted electronically the day services are provided for prompt processing. To allow us to be efficient on your behalf, we ask that you provide the following information to us prior to your first visit:
-
Name of the insured person (the parent or guardian with the insurance policy)
-
Date of birth and social security number of the insured person (Note: this information is strictly protected for your security. All written documents are shredded when no longer needed)
-
Employer name
-
Insurance group number
-
Insurance company name and phone number
-
Patient’s full legal name
-
Patient’s date of birth

It is essential for you to make sure your child is listed on your insurance policy prior to coming for the first visit.

Otherwise, you may not be eligible for any insurance benefits for the visit. It is easy to do so by calling the customer service number for your insurance company (NOT the human resources department of your employer). This is a great opportunity for them to be of service to you by explaining your particular plan and your benefits.
The amount of coverage varies widely among the different insurance plans.
Most insurance plans allow you to freely choose the dentist you care to see. Some plans, though, may try to encourage you to seek care at certain “in-network” offices. While your out-of-pocket costs may be lower, your ability to freely choose a qualified pediatric dentist may be restricted. Even if we are not in your network, we are still able to treat your children and process claims that are paid at the fee schedule of your plan.
Your out-of-pocket costs may be higher for an out-of-network provider. Contact customer service at your insurance company for information about your plan.
Payment Options
For your convenience, we accept payment directly from almost all insurance plans for completed treatment. You are responsible only for patient portions or procedures not covered by insurance, which may be paid with:
-
Check
-
Credit Card: Visa, Mastercard, or Discover (not American Express)
-
Debit Card
Payment can be prepaid before your appointment date or can be paid at the time of the appointment check-in.
Payments is required at the time of the visit.
